The Practice does not use this website to collect your protected health information (PHI), medical history, symptoms, diagnoses, or insurance details.

• We do not ask for insurance member IDs or insurance company names.
• We do not operate a public form that stores appointment or clinical information.
• We do not use Google Analytics, advertising pixels, Meta Pixel, Google Tag Manager, or session-replay tools on this website.

Information You Submit

Where a request option is offered, you may provide only basic contact details (such as your name and a telephone number or email) to ask for a general callback. Please do not include health information. We use that contact information only to return your message.

Third-Party Scheduling Links

Bookings are made through verified third-party scheduling providers. When you click a booking link you leave this website and are subject to that provider's terms and privacy practices. Current providers:

• Zocdoc — online appointment booking (zocdoc.com).
• Tebra — secure scheduling and, for existing patients, the patient portal (tebra.com).

Please review those providers' privacy policies. Any health information you share with them is governed by your relationship with that provider and the Practice, not by this website. You are not required to use these links to receive care — you may also call (305) 275-8573.

Hosting and Platform

This website is hosted on Base44 ("Base44"), which provides web application hosting, database storage, and email delivery. Base44 has access to technical/operational data (such as server logs) necessary to run the site. The Practice's use of Base44 for storing appointment or clinical information requires a signed Business Associate Agreement; currently the Practice does not store PHI submitted via public website forms in Base44.

How We Use and Disclose Information

• To return general callback requests and answer non-clinical questions.
• To operate and secure the website.
• We do not sell your information or use it for advertising.

Retention and Security

Basic callback contact details are retained only as long as needed to respond and for reasonable recordkeeping, unless a longer period is required by law. We use access controls to limit who can view stored information. Sensitive appointment and clinical records are maintained in the Practice's approved clinical systems, not through public website forms.

Your Rights and Requests
