This Notice of Privacy Practices describes how Integrative Pain Institute may use and disclose protected health information for treatment, payment, and healthcare operations, and your rights regarding that information. The full Notice applicable to your care is provided at the Practice and through its approved clinical systems.

Your Health Information Is Not Collected Through This Website

This website does not collect your medical history, symptoms, diagnoses, insurance member IDs, or other protected health information through public forms. Scheduling is performed through verified third-party providers (Zocdoc, Tebra). Clinical records are maintained in the Practice's approved clinical systems.

How We May Use and Disclose Health Information

  • Treatment: to provide, coordinate, and manage your care.
  • Payment: to bill and collect payment for services, as permitted.
  • Healthcare operations: for quality assessment and practice management.
  • As otherwise required or permitted by law, including certain public-health and safety situations.

Your Rights

  • To request access to and a copy of your records.
  • To request an amendment to your records.
  • To request restrictions on certain uses and disclosures.
  • To request confidential communications.
  • To receive an accounting of certain disclosures.
